Prior to attending a camp or clinic by Midwest Cheerleading, the following participation waiver must be completed. All liability and release forms will be collected upon our instructor’s arrival the FIRST MORNING of your scheduled camp (do not mail forms to our office). Any participant who fails to complete the necessary paperwork will be unable to participate until all information is gathered.
